Description:
Seeking an outgoing, dynamic individual responsible for developing competitive employment opportunities in the community through contacts with area employers. Will also complete job site analyses, monitor progress of new employees at their worksites and when necessary intervene to promote vocational success. The job developer must be knowledgeable of local labor trends, will create and update reports of progress and other documentation, promote community awareness of services through individual and group presentations. Description:
Supervise the day-to-day activities of the clients during shift; monitor clients taking medications; take vitals. SUMMARY The A.O. Chafee Specialist facilitates assigned young people’s transition to independence. The Chafee Specialist participates on the person-centered team that writes the Chafee Plan for Services and any program addendums and oversees implementation of the program and individual person served outcome goals. The Chafee Specialist will serve as a role model in taking positive action toward goals established and approach young people from strength’s based, positive manner. A friendly, cheerful and positive attitude is crucial to this role. The Chafee Specialist has responsibility to see the individual youth’s plan is implemented and to coordinate services to allow goals to be met. She/he will be responsible to coordinate the daily, weekly, monthly, quarterly or annual processes, develop support systems in the community, making appropriate community referrals, coordinate training and oversee skills training for the youth, as well as recording and tracking support documentation on on both hardcopies and database. She/he will also may participate in training of staff, and participate in interviewing and hiring new support staff and assist in managing support staff as requested by direct supervisor. ESSENTIAL DUTIES AND RESPONSIBILITIES Essential duties and responsibilities include the following. Other duties may be assigned. • Advocate for persons served • Work under the direction of the Program Manager and Direct Supervisor as a member of the team to create a positive welcoming environment in the living facilities • Process referrals, to include conducting intake/assessments, weekly, monthly and quarterly database entry and reporting • Ongoing work with youth for goal acquisition and support • Training and/or assisting youth with life skills development as specified on youth Individual Action Plan or Chafee Plan for Services, in collaboration with Family Support Team, case managers etc. • Works with the youth’s core team members to on plans for each youth and has primary responsibility to implement, document and suggest changes to the plan as the on-going assessment occurs ( must be strength - based, cover all relevant transition domains and focused on the future) • The planning process must identify obstacles to living independently, obtaining employment, obtaining post-secondary education, and in developing supportive social and recreational activities • Develop strategies to overcome barriers • Promote the involvement of appropriate others, informal and formal key players, in planning services and supports for each youth • Establishes a supportive relationship with each youth assigned by specialist in the program and is available to meet individually with each person contact or service 1) Chafee - a minimum of once a month and as necessary 2) Aftercare – immediate emergency response and as needed • Work flexible hours including nights, weekends and holidays as needed • Keeps case records up to date and maintains the case record confidentiality • Writes Progress Notes and other documentation as required for each youth served, creates monthly summaries for each youth to be sent to the referring agency, parents and shares this information with the youth • Enters information on youth into appropriate database system • Accompanies youth to important interviews, appointments, etc. • Physically able to observe, document, participate in activities with youth and staff and drive a car. • Supports youth to develop support systems in the community • Coordinates with community professionals as necessary to provide services • Adhere to all HIPAA and 42 CFR requirements • Must attend required training • Must follow all AO policies and procedures for best practices in conduction of youth services • Solicit in-kind and donations as needed • Must pass all background checks and sign documentation declaring past work experience with Children’s Division and other State Agencies. Minimum Qualifications:
The Chafee Specialist – preference of a Bachelor's Degree in the human service field or significant college hours in a human services field and three years experience with adolescent youth, residential treatment, or community-based services to children and adults.

Must have valid driver's license, acceptable driving record and current auto insurance.
Must participate in AO orientation training, specific foster youth program training and complete annual training requirements.
